Home and Community Care (HACC) is a cost shared program between the Commonwealth government and the State / Territory governments. It offers a wide range of regionally based services with the aim of preventing the inappropriate admission to healthcare facility care and improving the quality of life for frail aged people, people with disabilities and their carers.
