Short Description
Home and Community Care (HACC) is a cost shared program between the Commonwealth government and the State / Territory governments. It offers a wide range of regionally based services with the aim of preventing the inappropriate admission to healthcare facility care and improving the quality of life for frail aged people, people with disabilities and their carers.

The Program assists many frail older people and people with disabilities to live independently in their own homes and to participate in their communities.
HACC also helps the carers of these people through assistance with
caring tasks or by giving them respite. It can also help carers with
information, training and education.
Local service information is available through Commonwealth Carelink Centres. . The Centres act as a single point of contact for the general public, carers, service providers, GP's and other health professionals. They provide information about a range of community care services.

. Meals on wheels and other food services

. Community monitoring / alarm systems
Home Care Service clients are individually assessed for service, and priority is given to those in greatest need. Clients pay a contribution for the services they receive. People of low income may be eligible for a higher rate of subsidy
