A card issued to people with a permanent disability who require attendant care support. The card allows an accompanying companion to enter free of charge to participating venues and activities.

The card holder can present their Companion Card at any affiliated venue or event that charges an admission fee and the organisation or business should admit the card holder's companion at no charge.
The Companion Card Program is available to a person who meets all the following criteria
* Lives in NSW
* Has a significant and permanent disability
* Due to the impact of the disability the person is unable to participate at most community venues and activities without attendant care support and
* The need for this level of support will be life-long.
To Apply
An application form must be completed and can be obtained through the companion card program by phoning or downloading the application via the website.
The application form needs to be completed by the person applying for the companion card or their advocate. Once the form has been completed, the applicant can submit it via post to the Companion Card Program along with two identical passport quality/sized colour photographs.
Once the application has been approved, the applicant will receive a credit-card sized card with their details and photograph. The companion card can only be used by the card holder.
