Nextt Home and Community Care

Nextt is an Australian provider of in home care across disability, mental health and aged care sectors. Nextt delivers a comprehensive range of community support services, including personal and home care, respite and 24 hour support, case management/co-ordination of supports, specialised community access and social inclusion services for people with a disability and/or mental illness. These services are delivered in Queensland, New South Wales and Victoria.

* Services are offered to match the needs of clients, with the support of a team who take a multi-disciplined approach to co-ordinating support while ensuring the right skills and personalities to promote collaboration.
* Nextt provides flexible support options and is on-call 24 hours, 7 days a week

Disability Support Services
* Nextt's disability services are delivered in line with the Your Life Your Choice self-directed framework which promotes individual consumer choice and supports the maintenance of the family unit and chosen lifestyle through the development of (in consultation with the client), an individual's plan..

Nextt provides services through various government sponsored programs
* HACC - The Commonwealth HACC (Home And Community Care) Program provides services that support older people to stay at home and be more independent.
* DVA - The Department of Veterans' Affairs (DVA) provides specialised personal and home care to veterans, war widows and widowers.
* NDIA - It provides community linking and individualised support for people with permanent and significant disability, their families and carers.
* Home Care Packages Program - The objectives of the Home Care Packages Program are to assist people to remain living at home and to enable people to have choice and flexibility in the way that the consumer's aged care and support is provided at home.

Applications
* For government subsidised services, you will require an assessment by an Aged Care Assessment Team (ACAT).
