More Details

This government scheme aims to maintain the standard of medical devices available in Australia and to minimise the adverse effects of problems with devices. It provides an additional means by which unsafe products or procedures can be identified quickly so that appropriate action may be taken. It encourages voluntary cooperation between users, government and industry and should be used in conjunction with local reporting channels.

What is a therapeutic device
* Any material, instrument, machine, appliance, implant, or component of these used in the delivery of health care.

What should be reported
* Typical problems may include deficiencies in labelling, instructions or packaging, defective components, performance failures, poor construction or design, (anything that adversely affects the use of a device). Suggestions for rectifying or improving product performance would be appreciated.
* Reports are investigated by technical staff in conjunction with the responsible company. The reporter is informed of the outcome of the investigation.
Incident Reporting Forms are available online on the TGA website or from the Laboratories Branch of the Therapeutic Goods Administration.
