Home Care Packages are individually planned and coordinated packages providing four levels of care to help Australians remain living in their own homes. They are funded by the Australian Government to provide for people with complex care needs. There is no minimum age requirement. Individuals are assessed by a professional assessor using a nationally consistent assessment framework.

The Home Modification Information Clearinghouse (HMinfo) is a Commonwealth Government funded information service that helps people make informed choices about modifications to their home aimed at improving safety, ease of access and comfort for older people and people with disability. Home Modifications Australia (MOD.A) is the national body that represents home modification providers for people who are frail aged and people with disabilities. MOD.A previously operated as the NSW HMMS State Council.

A telephone information service providing consumers, prescribers and pharmacists with independent and accurate information about the safe and effective use of all medicines. Operates Monday to Friday 9am to 6pm EST for the cost of a local call. Information is also available on their website.

A Commonwealth government allowance paid to people with disabilities who cannot use public transport without substantial assistance and who meet the eligibility conditions. It is a payment to help with travel costs for work, study or looking for work.

The Transition Care Program is a government funded program that aims to help a person improve their independence and confidence after a hospital stay. It provides a package of services including low intensity therapy and personal and/or nursing care as part of an ongoing but slower recovery process. Transition Care service delivery is managed by the state and territory governments.
